Office Policies
Appointment
We are available to schedule your orthopedic appointments Monday through Friday, from 8:00 am to 5:00 pm. To schedule, email your request to ehery@floridaortho.com or fill out our online appointment request form.
What to Bring to your Appointment
- Your current valid insurance card
- Referral or Authorization from your PCP, if necessary
- Your Specialist Co-pay
- Medical History, including all current medications and dosage information
- All previous medical records, x-rays, CT scans, MRI scans or other tests relating to your current medical issue.
Arriving
For your first visit, please arrive at least 30 minutes before your scheduled appointment time in order to complete your registration paperwork.
Appointment Changes
If you must reschedule or cancel your orthopedic appointment, please call us as soon as possible so that we can give your time to someone else. Repeated cancellations, no shows or reschedules can adversely effect your ability to schedule with us in the future.
Prescription Policy
No prescription will ever be dispensed by this office until a patient has been seen by a physician in this office.
Prescription refills are handled daily from 9:00am to 4:00pm. Please have the name of your medication, dosage and pharmacy number and address available when requesting a refill. You should check with your pharmacy after 4:00pm to see if your refill is ready.
Remember to check your medication requirements prior to the weekend and holidays! We will not refill Rx after 12:00pm on Fridays.
Disability and FMLA Paperwork
Please allow at least 72 hours for paperwork to be completed. Please note, there is a $35 charge for all forms to be completed by our doctors and/or staff. This is to be paid prior to forms being completed.
